The name of the form that I got from the county clerks office was titled "Notice to Vacate Property". This form has worked well for me in avoiding eviction court. It is also good to talk to the county clerks office before delivering this form, just to be sure that you get everything perfectly correct. One little mistake, including how you deliver it, and the way that you calculate the number of days notice that you give may hurt you if it ends up in eviction court.
