The tenant's agent should provide the tenant with an inventory condition form, but I routinely send it to the tenant even if I'm the listing agent so I know that they've received it. Since I'm frequently the one to write the lease, it's easy to just include it on zip forms.
Perhaps I'm reading this a little differently. I think the question you may be asking is who gives the Inventory/Condition form to the Tenant to fill out. My practice as the agent representing the tenant I always give them the form even if the listing agent has provided. It is better to be safe than sorry. However, I do take it a step or two further. I ask the client to provide a copy of the Inventory to me to keep on permanent file in case they should lose it. And, I suggest they take photos of the items noted on the completed form and to also provide me with a copy of the photos. #justsayin'
The tenant is responsible for getting the form to the landlord. It states in the lease how many days the tenant has to get the filled out form back to the landlord. Typically 3-7 days. We as Realtors might want to suggest to our clients to forward a copy to us when sending to the landlord so we know the task was completed and have a copy. I also suggest taking photos of any items listed on the report and include when sending to the landlord.

The tenants or their Realtor should return the form within the timeline established in their lease contract. Make sure you get the Landlord or his Realtor to sign it as well. Keep a copy for your records as this form will be referenced at the end of your lease to determine the condition you are returning the residence versus how you recieved it. Your security deposit hangs in the balance.
The tenant is ultimately responsible for turning the inventory condition form into the landlord within the specified period in the contract but I always try to facilitate this so it gets returned in a timely manner if i'm representing the tenant. If i'm representing the landlord then I work with the other agent to be sure it's returned by their client in a timely manner.
The tenant since they’re the ones in the lease agreement with one of the Agreements being turn in a inventory form. See para 15B in the promulgated lease. :)
